Download ALL ADR Local Forms (LBFs)
Follow the instructions below to download all our ADR Local Bankruptcy Forms & the master Index:
1. Click on the icon below to download the forms to your computer. The file you will download is named MEDIATIONADRFORMS.EXE and is in a compressed format.
2. Once the MEDIATIONADRFORMS.EXE file has been downloaded to your computer, it must be expanded. DO NOT ATTEMPT TO VIEW THE COMPRESSED FILE, YOU MUST EXPAND THE CONTENTS OF THE FILE FIRST!
3. After downloading, place the file in a clean, empty directory and expand it by double-clicking on it from within Windows Explorer.
4. Many of the files downloaded from the expansion of Mediation_ADR_Forms.exe are WordPerfect (version 6,7,8) format. There are various file names (e.g., 203.wpd). A number of others, including the Application for Inclusion on the Register of Qualified Mediators (i.e., 201.pdf) are in a fillable PDF format for ease of preparation. 5. After verifying you can read the new forms, REMOVE ALL OLD FORMS FROM YOUR SYSTEM!! |
