Official Bankruptcy Forms

Official bankruptcy forms are approved by the Judicial Conference of the United States for use in all bankruptcy courts.  Commonly used official bankruptcy forms include the petition, all schedules, and reaffirmation agreement forms.

Local Bankruptcy Forms

Use of local bankruptcy forms (LBFs) is often required by the court's local rules.  Failure to use an LBF when required may result in a filed document being returned to the filer or stricken from the case record.
