Forms for Filing a Bankruptcy Case

Below are the forms required for individuals without an attorney to file a chapter 7 or chapter 13 case (including a joint case).

If you need to file a bankruptcy petition on an emergency basis (for example, to stop an imminent foreclosure), please contact the clerk's office at 503-326-1500 or 541-431-4000 for instructions.

  • Fill out these documents according to these instructions.
  • If filing with Public Document Upload or via fax, file as a single PDF separately from other documents.
  • If filing in paper (using the mail or court drop box), be sure to sign the documents in all the required places and bundle them together (with a rubber band, clip, etc.) separately from other documents.

Official Form 121, Statement of Your Social Security Numbers

  • To protect against identity theft, this form will not be viewable by the public.

Official Form 122A-1, Chapter 7 Statement of Your Monthly Income

  • Completion of Form 122A-1 will determine whether either of the other forms is required.
  • Combine any required 122A forms into a single PDF, or with a staple if filing in paper, and file separately from other documents.

  • To qualify to have the filing fee waived, your income must be less than 150% of the official poverty line (see "Related Links" on the linked page).
  • If filing via mail or drop box, you may file one of these applications or pay the full filing fee with a check or money order.
  • File separately from other documents.
