SIGNATURE REQUIREMENTS Documents filed with the court must be signed as required by Federal Rule of Bankruptcy 9011. To sign a document submitted using this service, add your signature to the document in the appropriate location using one of the following methods: Type your name following an "/s/" to indicate the typed name is a signature (for example, "/s/ Jane Doe") or Print the document, sign using an ink pen, and create a PDF of the signed document. CREDITOR MAILING LISTS Creditor mailing lists cannot be submitted on this page. Instead, use the Contact Us feature and type or copy the creditor list into the Message field. See Local Bankruptcy Form (LBF) 104 for more information. To add creditors to a case that has already been filed, you must file LBF 728 and pay the required fee (currently $34).
