New Forms for Chapter 11 Monthly Operating Reports and Post-Confirmation Reports

Effective June 21, 2021, the U.S. Trustees will require that certain debtors use new forms for chapter 11 monthly operating reports and post-confirmation reports.  The uniform forms, and instructions for their use and filing, are available on the USTP’s website at

Relatedly, the CM/ECF Rule 2015 Financial Report event has been retired, and the following events are now available for use under the Miscellaneous menu:

  • Small Business Monthly Operating Report
  • Chapter 11 Monthly Operating Report
  • Chapter 11 Post-Confirmation Report
